Clinical Study of Multiple Primary Gastric Cancers



Abstract
Multiple primary gastric carcinoma was investigated on 7 of 368 reesected stomachs at department of surgery, Hallym University Han-gang Sacred Heart Hospital during the past 10 years from January, 1984 to December, 1993.
The incidence of multiple primary gastric carcinoma was 1.90%. Male to female ratio was 1.3 to 1 with average age of 61.5 and that was 1.3 to 1 with average age of 54.2 in the total gastric carcinomas.
In general, the incidence ofmultiple primary gastric carcinoma was significantly elevated in early carcinoma compared to advanced carcinoma, but the incidence of the multiple primary gastric carcinoma in early carcinoma(1.49%) was not higher than
in
advanced carcinoma in this study.
The mjority of the lesions(6 of the 7 cases) were located in pyloroantrum and body. The lesions were composed of the well differentiated adenocarcinoma in 2 cases(28.6%), moderately differentiated adenocarcinoma in 1 case(14.3%), poorly
differentiated
adenocarcinoma in 3 cases (42.8%) and moderately defferentiated adenocarcinoma with poorly differentiated adenocarcinoma in 1 cases(14.3%).
The lesions of the 6 cases(85.7%) were intesitnal type carcinoma and only the lesions of the 1 case(14.3%) were composed of diffuse type carcinoma.
These results indicate that the whole stomach must be carefully examined to detect accessory carcinoma before gastric surgery, especially for intestinal type carcinoma in the older age group and that the surgeon must be aware of the possibility
of
multiple primary gastric carcinoma in subtotal or partial gatrectomy.
